Clearance and Impact Testing Information

Students must have online sport packet complete with parent and student signatures, Green emergency card filled out and brought to book room (library) for clearance signature, and current physical (dated after June 9th, 2017). Online clearance conformation and the above paperwork must be brought to ASB office to complete clearance process.

A physical completed after June 9, 2017 will be required to complete your clearance. If you are having trouble getting a physical from your Primary Care Physician, the We Care Health Center in Gilroy will take High School students for $25. (408) 842-0278 appointment necessary.
Please click here to download the Impact testing permission slip. Student-athlete must bring permission slip at the time of the test. Test must be taken before the ASB office clears student.
If you are interested in playing a fall sport next year and were impact tested BEFORE January of 2017, you must re-take your baseline impact test to participate in any fall sport. Students wishing to participate in any sport at CHS will need to complete the online registration (directions below). Returning athletes will need to log back on to the clearance process, click on his/her name, and fill out the health page again- all other information should be saved and automatically transferred over. Please make sure you choose the correct SCHOOL, SPORT, & YEAR.
Once the online registration is complete, a signed and stamped physician's form has been obtained, and you have printed out and signed the signatures document for the online athletic clearance process....Students must bring their paperwork to get a signature of clearance from the Library that he/she does not owe any fines/fees. Students will then bring their paperwork to the ASB office to complete the clearance process. The ASB office will print out the Emergency card that students must bring to their coach as a proof of clearance.
