Clearance and Sports Information


2024-25 athletes: Spring sport clearances (Baseball, Softball, Track & Field, Badminton, Boys Golf, Lacrosse, Gymnastics, Swimming & Diving, Boys Tennis, and Boys Volleyball) are now open!
This process must be done before February 3rd (the official day of tryouts) *Note- clearances will not be processed over the weekend for February 1st and 2nd*
Visit the Home Campus platform to start the clearance process.

Clearance eligibility will be based on 1st semester grades and Community Service Hours from the start of the year

(9th- 0 hours needed, 10th- 20 hours needed, 11th- 40 hours needed, 12th- 60 hours needed)

Students must also have a GPA 2.0 or above, no F's, and no more than 1 N in Citizenship to be eligible.  
 

Download the GUSD Physical Form to take with you to get your physical. Please ensure form is complete with exam date, physician's stamp and signature, and all other areas filled in. GUSD Policy- MD, DO, NP, or PA can only sign form. Forms signed by Chiropractors are not accepted.

Football athletes only: All student athletes who are participating in FOOTBALL must have a valid baseline impact test on file. Baselines are good for two years (tests typically taken in 9th and 11th grade). Students must bring their GUSD laptop if they have one and the signed permission slip.

For Summer & Off-Season Workouts

Students who would like to attend off-season workouts, open field/gym, or weights must print out and complete this waiver *:

* IMPORTANT: The waiver is only valid for off-season or summer workouts. Athletes who want to compete in fall, winter and/or spring sports need to complete the Home Campus clearance before their actual season begins.

For Fall, Winter & Spring Sports

Athletes competing for Christopher High School must complete the clearance process for each season by these steps:

  1. Complete the online clearance. Download instructions in English or Spanish.
  2. Get this physical examination form completed and dated AFTER June 1, 2024, and upload the form to Home Campus.
  3. Upload your physical form, proof of insurance, and signed confirmation page to Home Campus.
  4. Once the online clearance process is complete and all required paperwork is submitted, the CHS Athletic Department will finalize an athlete's clearance. Being cleared is not an instantaneous process. If your clearance has any issues, the Athletic Department will contact you.
  5. When cleared, you will receive an email with instructions to pick up your emergency card from the front office. All athletes should purchase an ASB Card ($50) when picking clearance paperwork.
