Work Permit Information

Please consider the following information when applying for a work permit:

  • Review the Student Work Permit Procedures
  • Completed application: Statement of Intent to Employ a Minor and Request for Work Permit Application, including: Minors Information 
  • Employer’s Information
  • Parent or legal guardian’s signature for approval to work.

*Request for Work Permit
Students may request a work permit from the school office coordinator. All students applying for a work permit must have a minimum of a 2.0 GPA. Students must also have good attendance and no disciplinary actions or incidents during the year. A work permit can be revoked if the student does not meet any and/or all of these requirements.

When school is in session, please submit your completed work permit request form to Kimberly Lemus, CHS Office Coordinator.  When school is not in session (summer), please email to kimberly.lemus@gilroyunified.org.  You must submit the following documentation:
